5年の経過を観察した胃潰瘍合併の多発性びまん性胃粘膜下囊腫の1例

A Case of Multiple Diffuse Cystic Disease of the Stomach

　従来胃囊腫は胃良性腫瘍の中でも稀な疾患とされ，文献上の報告も少なく，X線学，内視鏡学など進歩した昨今でも，その診断が必ずしも容易でない．最近われわれは興味ある臨床的および組織学的所見を示した多発性びまん性胃粘膜下囊腫を経験したので報告する．

　The patient (44-year-old man) visited our hospital for epigastralgia initiated in February 1972. Gastro-endoscopic and X-ray examinations revealed multiple submucosal tumors with alternating aggravations and remissions of gastric ulcer. In July 1977 subtotal gastric resection was performed for aggravated submucosal tumors. In the resected specimen were observed gastric ulcer scar in posterior wall of the lower corpus and a marked thickening of walls and multiple submucosal tumors in the lesser curvature from corpus to antrum. The cysts were divided histologically into two kinds localized in the submucosa: one was polylocular multiple cysts comprising a layer of cylinderical or cubical epithelia and the other was monolocular multiple cysts with such a structure evoking gastric mucosa and muscularis mucosae.

　By these findings we were able to diagnose this case as multiple diffuse cystic disease of the stomach.
