 A 63-year-old complaining of high fever and burning pain in the epigastium was admitted to our hospital in May 1974, and a diagnosis of septic infection was made. Laboratory findings showed positive CRP and marked leukocytosis. He became afebrile after antibiotic therapy with ampicillin.

 Roentogenogram demonstrated a large deep ulcer at the distal end of the esophagus. Esophagoscopy revealed a large deep ulcer with white coat and fragile protruded margins.

 Operation was performed on Feb. 12, 1975. The partially excised esophagus and stomach revealed a large penetrated ulcer at the end of the esophagus, measuring 3×3×0.8 cm, covered by the minor omentum.

 After operation he became quite well. The esophageal. ulcer was thought to be the primary focus of septic infection.
