短期間に形態を変えた胃平滑筋肉腫の1例

The Leiomyosarcoma of the Stomach Changing Its Shape in a Short Time, Report of a Case

要旨　胃粘膜下腫瘍の経過観察中に胃X線写真上，腫瘍径が24×20mmから32×28mmに増大し，1年6か月後には中心陥凹も形成された．手術の結果は平滑筋肉腫であり，病理組織学的に潰瘍底に壊死が認められた．このことより，腫瘍の発育と共に腫瘍中心部の壊死が大きくなり，その壊死が胃粘膜表面に達して中心陥凹が形成されたものと推定された．

　The patient was a 40-year-old gentleman. X-ray examination revealed a smooth-surface protruding lesion with bridging folds on the posterior wall of the upper gastric body, on May 1981. His case was followed-up, and on November 1982, a double contrast film showed a central ulceration. Local excision was performed on December 24, 1982. Histological study of the resected specimen revealed leiomyosarcoma of the stomach (intramural type).

　These findings may suggest that the necrosis within the tumor due to its enlargement caused a central ulceration.
