要旨 1832年にHodgkinがHodgkin病を記載し,更にSternbergが1926年当時の造血組織の腫瘍を分類してから,現在最も新しく,しかも最も当を得た非Hodgkinリンパ腫の組織学的分類と思われるUpdated Kiel分類に至るまでの,臨床医や病理医に少なからぬ影響を与えた代表的な悪性リンパ腫の分類について述べた.同時にそれぞれの分類の基本的な考え方についても述べた.それらの変遷は主としてリンパ球の形態と機能についての研究の進歩によるものであった.また,最近盛んに取り沙汰されている特殊な節外性リンパ腫,例えばmucosa-associated lymphoid tissue(MALT)リンパ腫の取り扱い方についても筆者の見解を述べた.
Since the original report on patients with lymph node swelling by Hodgkin in 1832, several representative classifications of malignant lymphomas (MLs), which have had a strong impact on clinicians and pathologists, have been proposed, from the classification of tumors by the hematopoietic system of Sternberg in 1926 to the updated Kiel classification of non-Hodgkin's lymphomas (NHLs) in 1988. This latter classification is the most recently proposed system and is considered to be the most reliable. The basic standpoints of these classification systems are briefly discussed. A major cause for change in the classification of MLs was the development of studies on the correlation between lymphocyte morphology and immunological function at each stage. Finally, the authors give their opinion regarding several special types of extranodal MLs, e.g. mucosa-associated lymphoid tissue (MALT) lymphoma.
