 Chronological changes in the morphologic features of gastric cancer and associated treatment modalities and their long-time results were reviewed through the analysis of 10,486 cases accumulated over the past 45 years at the Surgery Division of the Cancer Institute Hospital, Tokyo. Extent of cancer spread has shifted to earlier stages, with an increase of cancer located in the upper part of the stomach, and an increase in signetring cell cancer. The rate of curative surgery and fiveyear survival rates increased by twice during the past 45 years. The latest data show the rate of curative resection to be 84.5%, five-year survival rates to be 75.9%, and direct mortality after surgery to be 1.3%. Patients with Stage Ⅱ or more advanced disease should be treated with wide resection and systemic lymphatic dissection with curative intent, followed by adjuvant chemotherapy.
