要旨 オキシドールを浣腸して生じた急性出血性大腸炎を経験した.患者は67歳の男性.便秘がありオキシドールを浣腸したところ,肛門出血・便意頻回・残便感を生じた.発症後3日目に行った大腸内視鏡検査では,歯状線から35cmまでの直腸・S状結腸に粘膜の著明な発赤・浮腫を認め,下部直腸にはびらん・出血が見られた.7日目には症状はほぼ消失し,17日目の大腸内視鏡検査では,粘膜の発赤・浮腫は改善しており,再生上皮を伴った浅い縦走潰瘍瘢痕が見られた.25日目にはほぼ正常の粘膜となった.薬剤を注腸して発症した大腸炎はまれであり,内視鏡所見を呈示し報告した.
A 67-year-old man was admitted with complaints of anal bleeding, frequent desire defecation, and sensation of remnant stool. He had suffered from constipation, and had given an hydrogen peroxide enema by himself. Colonoscopic examination demonstrated marked redness and edema of the rectosigmoid colon, and erosion and bleeding in the distal side of the rectum. Two weeks later, colonoscopic examination showed an improvement of redness, and shallow longitudinal ulcer scars with regenerative mucosa. On the 25th day, colonoscopic examination revealed almost normal mucosa. Hemorrhagic colitis induced by hydrogen peroxide enema is very rare, its endoscopic findings are characteristic and it was cured without surgical intervention.
