ヨード不染病巣の経過追跡例に認めた食道表在癌の2例

The Growth and Development of Superficial Esophageal Cancer Arising from Lesions Unstained by Iodine, Report of Two Cases

要旨　アルコール依存症患者の食道全域に多発するヨード不染病巣の経過追跡例において，下咽頭と食道に異時性重複癌の発生を認めた1例と，アカラシア患者に対しての術後6年間にわたる経過観察で食道表在癌と診断された1例を経験した．内視鏡での経過観察を中心に，病巣部での粘膜面の変化，ヨード染色での不染部の色調や形態の変化，また生検での病理診断の困難性と問題点につき言及した

　We observed two cases of superficial esophageal cancer which developed in follow-up lesions which were unstained by iodine.

　(Case 1) was an alcoholic, chromo-endoscopy for esophageal cancer screening revealed multiple tiny unstained lesions throughout the entire esophagus. Hypopharyngeal and esophageal cancers were discovered within a two-year period by the before-mentioned screening method.

　(Case 2) had had achalasia for 30 years and underwent an operation six years previously. Follow-up endoscopic examinations for six years revealed the development of superficial esophageal cancer.

　We mainly observed the growth and development of superficial esophageal cancer by endoscopy and encountered difficulty in pathological diagnosis from biopsy specimens.
