胃原発mantle cell lymphomaの1例

Mantle Cell Lymphoma of the Stomach, Report of a Case

要旨　患者は62歳，男性．1999年4月心窩部痛を主訴に当院を受診した．上部消化管内視鏡検査にて胃に不整潰瘍を伴う巨大皺襞を認め，生検で悪性リンパ腫と診断した．胃全摘術を施行し，病理組織診断ではmantle zone由来の中型リンパ球様細胞のびまん性浸潤を認め，それらは免疫組織化学にてCD5とcyclin D1に陽性であり，mantle cell lymphomaと診断した．術後5クールのCHOP療法を行い経過観察中である．

　A 62-year-old man visited our hospital with epigastralgia. Upper gastrointestinal endoscopy revealed that the folds were swollen and ulcerative lesions were present in the posterior wall of the middle body of the stomach. Malignant lymphoma was diagnosed by endoscopic biopsy. The patient underwent total gastrectomy. Histopathological diagnosis by H・E and immunohistochemical stain (cyclin D1) was compatible with mantle cell lymphoma. The patient was given five courses of anticancer chemotherapy and is still well 10 months after the operation.
