Surgical Treatment for Superficial Esophageal Cancer

要旨　食道表在癌の治療に際しては占居部位，壁深達度や脈管侵襲の有無，リンパ節の画像評価などを詳細に検討したうえで治療法を選択しなければならない．T1a-MM以深の表在癌に対する根治手術では，どの術式においても局所制御を高めるための精度の高いリンパ節郭清術が求められる．これまでの検討では食道表在癌のセンチネルリンパ節は頸部から腹部まで広範囲に分布しており，センチネルリンパ節の一部が3群リンパ節以遠に分布する症例が約30％の症例に認められていた．このセンチネルリンパ節分布の複雑さは，食道壁内，縦隔の複雑なリンパ流に起因するものであり，頸胸腹3領域リンパ節郭清術の妥当性を示していると言える．

　The specific characteristics of esophageal cancer is multi-directional lymphatic flow from the primary lesion, and wide spread and random patterns of lymph node metastasis from the cervical to the abdominal areas. Based on these clinical observations, extended radical esophagectomy with three-field lymph node dissection has become recognized as a standard surgical procedure in Japan. Esophagectomy with three-field precise lymphadenectomy may be a reasonable procedure even for esophageal superficial cancers because of the wide distribution of sentinel lymph nodes and unpredictable metastatic patterns.
