Treatment of Superficial Esophageal Cancer: Endoscopic Submucosal Dissection

要旨　食道ESDは病変の占居部位，大きさにかかわらず一括切除が可能な優れた治療手技だが，食道壁は薄く，心拍動で常に動いているため技術的なハードルが高い．上下アングルのみならず左右アングル操作も左手のみで操作できる技術を身につけたうえで，各種デバイスや高周波装置の特性を熟知した術者のみが施行すべきである．安全なESDを施行するためには，直視下での切開剝離操作が必須であり，良好なカウンタートラクションを得るためには重力の利用や，糸付きクリップ法，トンネル法などが有用である．

　ESD（endoscopic submucosal dissection）is a useful treatment method for superficial esophageal cancer. However, the muscle layer of the esophagus is thinner than that of the stomach, so the risk of perforation is higher than in gastric ESD. Therefore, the operator has to have enough skill to control the scope during ESD. The skill of marking, submucosal injection, mucosal incision, submucosal dissection and hemostasis have been described in this paper. Direct observation of the submucosal layer is the most important point for the prevention of complications such as bleeding or perforation. The tunnel method and the clip-with-line method are useful techniques for making good counter traction during ESD.
