低異型度分化型胃癌の病理学的特徴―腺腫との鑑別を含めて

Pathological Characteristics of Low-Grade Differentiated-type Adenocarcinoma of the Stomach

要旨　胃の分化型癌には“低異型度高分化腺癌"あるいは“超高分化腺癌"などと呼ばれ，細胞異型が乏しく分化傾向が明瞭なものが存在する．このような癌には，臨床的に明らかな癌ととらえることができるにもかかわらず，病理医が癌と診断しえないものや画像的にも癌の存在・範囲診断が困難なものが含まれ，しばしば臨床病理学的な問題症例とされる．病理学的には，正常，再生・過形成あるいは腸上皮化生との鑑別が問題となるものと，腺腫のスペクトラムに含まれるようなものがある．臨床医，病理医ともこのような病変の存在を認識，理解し，お互いの診療能力を高めていくことが重要である．

　There are some gastric adenocarcinomas showing very low-grade nuclear atypia and clear cellular differentiation. They have been called“well differentiated adenocarcinomas with low grade atypia"or“extremely well differentiated adenocarcinomas", which is sometimes difficult for general pathologists. to diagnose as cancer. From the histopathological point of view,“low-grade differentiated-type adenocarcinomas"are categorized as adenocarcinoma with very similar to normal, regenerative, hyperplastic or intestinalized epithelium, but in the spectrum of adenoma. Both clinicians and pathologists should recognize and understand the characteristics of this disease entity.
