スキルス胃癌と鑑別を要する非腫瘍性疾患―サルコイドーシス

Sarcoidosis

要旨　サルコイドーシスは原因不明の全身性肉芽腫性疾患であるが，消化管に発生することはまれである．胃サルコイドーシスには，全身のサルコイドーシスの部分症として生じるものと，胃に限局して生じる限局型サルコイドーシスがある．大部分がこの限局型である．胃サルコイドーシスの肉眼所見は，多発潰瘍やびらん，スキルス胃癌を疑わせる粘膜の肥厚や硬化，結節性隆起性病変などが挙げられるが，多彩であり，特異的なものはない．今回報告する症例は前庭部型のスキルス胃癌との鑑別が必要であり，ステロイド治療にて症状，所見の明らかな改善を認めている．

　Sarcoidosis is a unidentified systemic granulomatous disease, but gastrointestinal involvement in sarcoidosis is rare and, when it does occur, it occurs in the stomach rather than in other digestive organs. It is assumed that this limitatiom to the stomach is a symptom of sarcoidosis of the whole body. Stomach sarcoidosis is divided into the following categories : gastric ulcer type, diffuse infiltrating carcinomas or Menetrier's disease type, and polypoid lesion type. Skill is needed to discriminate this type of stomach sarcoidosis from infiltrating vestibular carcinomas. It is clear also that steroid therapy brings about improvement to this sarcoidosis limited to the stomach.
