欧米におけるcolitic cancer/dysplasiaの診断・治療―現状と問題点

The Present Status and Problems in Diagnosis of Colitic Cancer/Dysplasia, with a Review of European and American Literature

要旨　colitic cancer/dysplasiaの診断と治療の現状と問題点に関する欧米の文献をレビューし総説した．挙げられる問題点は，（1）サーベイランスプログラムが各ガイドラインで異なっている，（2）サーベイランスにおいてガイドラインが提唱する生検個数が遵守されていない，（3）各サーベイランスの有効性を立証する直接的証拠が示されていない，（4）INDとLGDの診断において診断者間でかなりの不一致がみられる，（5）平坦型LGDの取り扱いに関して統一見解が得られていない，（6）DALMの定義に解釈のずれがある，（7）DALMとALM/ALD，散発性腺腫，炎症性ポリープの鑑別が難しい例があり，適切な鑑別方法が見い出されていない，などである．

　The European and American literature about the present status and problems of diagnosis and the treatment of colitic cancer/dysplasia were reviewed.

　The problems that arise are as follows,（1）The surveillance program is different in each guideline,（2）The biopsy number that the guideline proposes in surveillance is not observed,（3）Direct evidence to prove efficacy of each surveillance is not shown,（4）Significant disagreement is thought to exist between diagnosticians in the diagnosis of LGD（low-grade dysplasia）and IND（indefinite dysplasia）,（5）A common view is not obtained about the handling of flat LGD,（6）There is divergence in the interpretation of a definition of DALM（dysplasia-associated lesion or mass）,（7）There are cases where the differentiation of DALM,ALM（adenoma-like mass）/ALD（adenoma-like dysplasia）, sporadic adenoma, and inflammatory polyp is difficult, and an appropriate differentiation method has not been found.
