要旨　隆起型早期胃癌36例，胃腺腫34例，腺腫内癌14例に対し，NBI併用拡大内視鏡を施行し，表面血管，表面構造について検討した．血管パターンは網目，楕円，不規則血管，表面構造は敷石状構造，管状構造，小型敷石状構造，大小不同敷石構造に渦巻血管を伴うものに分けられた．隆起型早期胃癌の高分化腺癌では網目血管36％，敷石状構造50％，管状構造6％にみられ，敷石状構造に代表される表面構造が主体であった．胃腺腫では敷石状構造38％，管状構造29％であり，網目血管を示すものは21％であった．腺腫内癌では網目血管36％，敷石状構造50％であったが　副所見も含めると79％に網目構造がみられた．腺腫内癌の術前診断にNBI併用拡大観察を施行し，網目血管の所見が有用であると考えられた．

　36 cases of elevated early gastric cancers and 34 cases of adenomas were examined concerning surface microvascular structure and surface structure using magnified NBI.

　Elevated early gastric cancers showed a fine network surface microvascular structure and surface structure like cobblestone. 36％ of cases showed a fine network pattern. 50％ cases showed cobblestone-like surface structure and6％ of cases showed tubular surface structure. On the other hand, adenoma showed cobblestone-like surface structure (38％), tubular surface structure (29％) and fine network pattern (21％). Cancer with adenoma showed fine network pattern (35％) and cobblestone-like surface structure (50％). The main surface structures are fine network pattern and cobblestone-like surface structure in cancer with adenoma. But fine network pattern was observed in 79％ of cancer cases with adenoma as minor findings. Fine network pattern is a finding which is useful in the differential diagnosis adenoma and cancer with adenoma.
