 To delineate the genetic pathways in the clonal evolution of flat colorectal cancers, multiple foci were microdissected and loss of heterozygosity (LOH) analyses were performed. Some cases showed homogeneous LOH of multiple chromosomal arms throughout the foci, indicating possible de novo type carcinogenesis. Other cases showed clonal genetic progression and/or divergence in the mucosa, with some foci evolving into sm invasion. High incidence of homogeneous 17p LOH concomitant with positive p53 immunohistochemical staining suggests early involvement of p53 abnormality. Homogeneous LOH of 3p was detected in 32%, indicating early involvement of the gene in 3p in a subset of tumors. Genome wide LOH analysis together with investigation of specific genetic abnormality in comparison with histology may reveal unique genetic pathways of flat colorectal tumors which are different from conventional adenoma-carcinoma sequences.
