炎症性腸疾患と鑑別困難な感染性腸炎の診断と経過―Crohn病との鑑別を中心に

Infectious Diseases which should be Differentiated from Crohn's Disease

要旨　Crohn病を感染症と誤診した場合に比べて，その逆の場合は重大な帰結をもたらす．Crohn病と鑑別を要する感染性腸炎として代表的なものは，腸結核，アメーバ性大腸炎，エルシニア腸炎である．特に腸結核とアメーバ性大腸炎は慢性経過を辿り診断に迷うことがある．Crohn病の典型像である縦走潰瘍や敷石像を呈する感染症は極めてまれであり，感染性腸炎とCrohn病の鑑別が問題となるのはCrohn病としては非定型的なアフタや方向性不明の多発性潰瘍がみられる場合がほとんどである．感染症を疑っていても病原体が証明できない場合に安易にCrohn病と診断してしまわないよう注意が必要である．

　It is much more harmful for patients if infectious diseases are misdiagnosed as Crohn's disease than vice versa. Infectious diseases which need differential diagnosis from Crohn's disease include tuberculosis, amebiasis, and Yersinia enterocolitis. Tuberculosis and amebiasis are especially important because of their chronic course. Longitudinal ulcer or cobblestone appearance is rarely observed in infectious diseases, so puzzling conditions occur when aphthae or ulcers are found distributed in obscure directions. When infectious diseases are suspected, care should be taken not to easily make the diagnosis of Crohn's disease just because infectious agents cannot be demonstrated.
