We describe the point of diagnosis, endoscopic characteristics, and cases of Yersinia enterocolitis, intestinal tuberculosis, amebic colitis, and CMV(cytomegalovirus)enterocolitis that present endoscopic diagnostic challenges. The endoscopic characteristics of Yersinia enterocolitis, intestinal tuberculosis, and amebic colitis are mostly distinct;however, a few cases are difficult to diagnose by endoscopy. Endoscopic findings of CMV enterocolitis vary;therefore, CMV enterocolitis should be suspected not only by endoscopic findings but also on the basis of patient history. Detection of pathogenic microorganisms is required to diagnose infectious enterocolitis, and the combination of stool culture, histologic culture, serum antibodies, and serum antigens is necessary for a definite diagnosis. We should consider therapeutic diagnosis in the situation of a severe disease as well as diagnosis by endoscopic or other findings.
