診断困難な感染性腸炎

Infectious Enterocolitis with Endoscopic Diagnostic Challenges

要旨●エルシニア腸炎，腸結核，アメーバ性大腸炎，サイトメガロウイルス（CMV）腸炎の診断のポイントと内視鏡的特徴像について述べ，非典型的内視鏡像を示す診断困難例について述べた．エルシニア腸炎，腸結核，アメーバ性大腸炎の内視鏡的特徴像はほぼ明らかとなっているが，それでも診断困難例は存在する．CMV腸炎の内視鏡像は多彩であり，内視鏡像のみでなく患者背景から疑う必要がある．それぞれの疾患について診断困難例を提示した．感染性腸炎は実際に病原体が検出されなければ診断できないため，疑わしいが確定診断できない症例が一定数存在する．便培養，生検培養，生検検査，抗体検査，抗原検査などを組み合わせて診断することが重要である．確定診断がつかなくても，内視鏡所見や他の検査所見で確実と思われる場合や重症で生命が危険と思われる場合は治療的診断の適応がある．

　We describe the point of diagnosis, endoscopic characteristics, and cases of Yersinia enterocolitis, intestinal tuberculosis, amebic colitis, and CMV（cytomegalovirus）enterocolitis that present endoscopic diagnostic challenges. The endoscopic characteristics of Yersinia enterocolitis, intestinal tuberculosis, and amebic colitis are mostly distinct；however, a few cases are difficult to diagnose by endoscopy. Endoscopic findings of CMV enterocolitis vary；therefore, CMV enterocolitis should be suspected not only by endoscopic findings but also on the basis of patient history. Detection of pathogenic microorganisms is required to diagnose infectious enterocolitis, and the combination of stool culture, histologic culture, serum antibodies, and serum antigens is necessary for a definite diagnosis. We should consider therapeutic diagnosis in the situation of a severe disease as well as diagnosis by endoscopic or other findings.
