Acetic Acid, Indigocarmine Dye Spraying and Water Lavage─Its Usefulness Indetermining the Horizontal Margin of Early Gastric Cancer

要旨　早期胃癌に対する内視鏡治療の適応拡大に伴い，病変の側方進展範囲診断はますます重要性を増している．当院では通常の内視鏡機器を用いて，酢酸・インジゴカルミン撒布後に病変部を水洗することで，早期胃癌の側方進展範囲診断を行ってきた．この方法では腫瘍部のインジゴカルミンが水洗で除去されるのに対し，非腫瘍部のインジゴカルミンは残存するため，腫瘍の進展範囲が明瞭に描出される．酢酸・インジゴカルミン撒布・水洗法は特殊な機器を必要とせず，腫瘍進展範囲の全体像が簡便かつ明瞭に認識できる手法である．自験例では正診率が94.9％と高く，有用な手法と考えられた．

　The importance of diagnosis of the horizontal margin of early gastric cancer is increasing as the indication of endoscopic resection for gastric cancer is widened. We have determined the precise horizontal margin of early gastric cancer by using acetic acid, indigocarmine, and water which are easily obtained, water, and requires no special equipment. Using this method, we can determine the precise horizontal margin of early gastric cancer because indigocarmine is washed out by water from the cancerous mucosa and remains in the noncancerous mucosa. This technique is one in which no special equipment is needed. Using this method it is easy to visualizing the whole image within the horizontal margin of early gastric cancer. This method is a useful technique because our diagnosis has been correct in 94.9% of cases.
