腸管の狭窄を来し膵浸潤を認めた十二指腸原発follicular lymphomaの1例

Primary Follicular Lymphoma of the Duodenum Presenting as a Mass Invading the Pancreas, Report of a Case

要旨　患者は57歳，女性．上腹部不快感を主訴に当科を紹介受診した．X線検査では十二指腸下行脚に約5cmにわたる全周性の狭窄が描出され，粘膜面には小潰瘍像が指摘できた．内視鏡検査では，上十二指腸角に小潰瘍が存在し，下行脚の全周性狭窄に続いていた．粘膜面には白色微細顆粒状の多発隆起所見がみられた．超音波内視鏡検査では腫瘍は不均一な点状エコーを呈する類円形の腫瘤として認められた．腫瘍を含めた幽門輪温存膵頭十二指腸切除術を施行した．切除標本では全周性で4.2×4.0cm大の粘膜下腫瘍性病変が十二指腸下行脚に存在し，粘膜面には小潰瘍が多発していた．病理組織学的には十二指腸壁の一部粘膜を除きほぼ壁全層に中型異型リンパ球の濾胞状増生が認められ膵頭部に浸潤しておりfollicular lymphoma，grade 1と診断した．病期分類Stage IVであり，術後抗CD20抗体（Rituximab）を併用したCHOP療法を行った．十二指腸原発follicular lymphomaの中で，腫瘤を形成し浸潤傾向を呈する進行した症例はまれであり，十二指腸原発follicular lymphomaの発育進展を知るうえで貴重な症例と考えられた．

　We report the case of a 58-year-old woman with primary follicular lymphoma of the duodenal 2nd portion presenting as a 5cm sized mass. Endoscopically and radiographically, the patient was thought to have a large tumor in the duodenum. Histologic and immuno-histochemical examination of the specimen from a pylorus-preserving pancreaticoduodenectomy procedure revealed follicular lymphoma, grade 1. This tumor originated in the duodenal 2nd portion and spread involving the entire wall of the duodenum and had invaded the head of the pancreas. Our case indicates that this type of follicular lymphoma can occur in this location and present with features of an advanced case.
