治療的角膜移植として角膜内皮細胞移植を施行した4例

Four cases of endothelial keratoplasty performed as therapeutic corneal transplantation

要約　目的：角膜内皮移植術は一般に視力改善を目的とした手術として理解されているが，視力改善が期待できない症例における治療的適応については十分に整理されていない。本研究では，疼痛軽減および感染管理を主目的として角膜内皮移植術を施行した症例を報告し，その意義を検討した。

対象と方法：視力改善を主目的とせず，Descemet stripping automated endothelial keratoplasty（DSAEK）を施行した水疱性角膜症4症例を後ろ向きに検討した。原疾患，術前後の視力，角膜所見，疼痛の有無を評価した。

結果：全症例で術後に角膜浮腫の改善と眼痛の消失を認めた。一方，術後視力の改善は限定的であったが，縫合を伴う従来の角膜移植術と異なり，角膜形状変化や不正乱視は認めなかった。

結論：角膜内皮移植術は，視力改善が期待できない症例においても，疼痛軽減および感染管理を目的とした治療的角膜移植として有用な選択肢となりうる。

Abstract　Purpose：To evaluate the role of endothelial keratoplasty as a therapeutic procedure in patients for whom visual rehabilitation was not the primary goal.

Subjects and Methods：Four eyes with bullous keratopathy underwent Descemet stripping automated endothelial keratoplasty primarily for pain relief and infection control. The clinical backgrounds, visual acuity, corneal findings, and postoperative symptoms were retrospectively reviewed.

Results：Corneal edema improved and ocular pain resolved in all cases after surgery. Postoperative visual acuity remained limited, as preoperatively anticipated；however, suture-related corneal deformation and irregular astigmatism were avoided.

Conclusions：Endothelial keratoplasty may serve as a therapeutic corneal transplantation option for symptom control and ocular surface stabilization in selected cases in which visual improvement is not expected.
