Abstract Case:A 70-year-old man had been experiencing transient vision loss in the left eye and headache for 3 years. A previous ophthalmologic evaluation revealed mild elevations in serum complement and C-reactive protein levels;however, non-contrast magnetic resonance imaging(MRI) of the head showed no obvious lesions. At the initial examination, the best-corrected visual acuity was 1.5 in the right and 0.8 in the left eye, with critical flicker frequency values of 34.3 Hz in the right and 12.2 Hz in the left eye. A visual field defect bordering the vertical meridian was observed in the left eye. The neuroretinal rim was preserved;however, optical coherence tomography was performed to rule out glaucoma, revealing no nerve fiber bundle defects or temporal raphe signs in either eye. Contrast-enhanced MRI of the head showed thickening of the left frontal fossa and left temporal dura mater with a contrast effect. The thickened dura mater compressed the left optic nerve at the transition between the optic nerve canal and the intracranial region, with a contrast effect observed around the optic nerve in the left optic nerve canal. The patient was admitted to the neurology department of our hospital for further evaluation. Blood and cerebrospinal fluid tests were performed, and the patient was diagnosed with idiopathic hypertrophic dura materitis. Although steroid pulse therapy was planned, visual acuity loss and visual field defects in the left eye improved without any treatment during the examination period. Consequently, the patient was discharged and followed up in the outpatient clinic.

Conclusion:This clinical case highlights a patient with recurrent visual loss and headache in the left eye, where contrast-enhanced MRI showed hypertrophic dura materitis that resolved spontaneously.
