Objective: This study aimed to clarify the current practices of occupational therapy (OT) and occupational therapists' perceptions regarding Adolescents and Young Adults (AYA) with cancer in Japan. Methods: A nationwide survey was conducted targeting occupational therapists working at designated cancer care hospitals, regional cancer care hospitals, pediatric cancer centers, and national pediatric cancer centers in Japan. Results: Occupational therapists engaged in cancer care primarily provided inpatient interventions focusing on physical function. However, many therapists recognized the need to address areas that directly impact post-discharge life, such as instrumental activities of daily living (IADL), educational support, and vocational rehabilitation. A gap was observed between current practices and therapists'perceptions. Conclusion: Since IADL, education, and employment are often meaningful occupations for AYA cancer patients, the development of continuous support systems in collaboration with community and industry sectors is warranted.
