Extracorporeal Membrane Oxygenation in Difficult Airway Management Strategies：Preface and Comments

　体外式膜型人工肺（extracorporeal membrane oxygenation：ECMO）は，近年ではCOVID-19パンデミックで注目され，麻酔科医にとってより身近なデバイスとなっているが，困難気道管理（difficult airway management：DAM）戦略においてもその存在感を増していることが，近年の麻酔科領域の学術集会における症例報告数からもうかがえる。そして，ECMOを用いた心肺蘇生（extracorporeal cardiopulmonary resuscitation：ECPR）や重症呼吸不全におけるECMO管理とは別に，DAMの特性を理解して正しくECMOを活用する能力が麻酔科医に求められている。この特集は “DAM戦略におけるECMO” と題し，日本麻酔科学会第71回学術集会（2024年）において同タイトルで開催されたシンポジウム内容を論文化するものであり，シンポジストを務めた4名のスペシャリストがそのまま執筆者となっている。これまでDAMについてはさまざまなテーマで議論が重ねられてきたが，DAM戦略におけるECMOにフォーカスして体系的に論述された企画は，前述のシンポジウムと本特集が最初と思われる。

　Anesthesiologists have become more familiar with the use of extracorporeal membrane oxygenation（ECMO）in difficult airway management（DAM）strategies, and they are now expected to understand the unique characteristics of DAM and to use ECMO appropriately in this context. This ‘ECMO in DAM Strategies’ special issue is based on a symposium held in 2024 at the 71st Annual Meeting of the Japanese Society of Anesthesiologists, with four specialists who served as symposium speakers contributing as authors. While DAM has been discussed from various perspectives, this special issue provides the first attempt to systematically address ECMO use within DAM strategies. The symposium’s main focus was on indications for ECMO in DAM, highlighting the challenges that lead anesthesiologists to hesitate or opt for ambiguous ‘ECMO standby’ strategies. The shared message from all contributors is clear：hesitation regarding the indications for ECMO can be detrimental in DAM. In addition, safe and effective uses of ECMO in DAM require multidisciplinary team-based care. This issue provides practical insights and addresses challenges regarding ECMO in DAM, encouraging anesthesiologists to refine both their individual judgment and institutional systems.
