DAM戦略における周術期ECMO

Perioperative Extracorporeal Membrane Oxygenation（ECMO）in Difficult Airway Management（DAM）

はじめに

　周術期における体外式膜型人工肺（extracorporeal membrane oxygenation：ECMO）の立ち位置はどこにあるのか。周術期，つまり術前から術中，術後におけるECMOの使用は，条件と適応がそろえば症例は少ないながらも存在する。

　ECMOは主に呼吸と循環両方をサポートするveno-arterial ECMO（V-A ECMO）と，呼吸をサポートするveno-venous ECMO（V-V ECMO）に大別される。困難気道管理（difficult airway management：DAM）戦略における周術期ECMOの多くはV-V ECMOが使用されると思われる。術前・麻酔導入前からのV-V ECMO導入や術中呼吸・循環管理，抗凝固，出血，感染などの周術期管理について解説する。

　周術期に限らず，ECMOは血管穿刺・カニューレ挿入から維持管理において危機的合併症を生む可能性があり，そもそも導入が必要なのか，必要ならばどのように管理すべきなのか，リスクベネフィットを十分勘案して使用するべきであることをまずは強調したい。

　Veno-venous（V-V）extracorporeal membrane oxygenation（ECMO）is frequently performed as perioperative ECMO together with the difficult airway management（DAM）approach. This article explains the introduction of V-V ECMO before anesthesia induction, intraoperative ECMO management, perioperative respiratory and circulatory support, and the risks of anticoagulation, bleeding, and infection and techniques to avoid these complications.
