Variants of ovarian mature cystic teratomas：a review of imaging findings

成熟奇形腫は2～3胚葉由来の成熟した体細胞組織で構成される良性腫瘍で，内部に皮膚（角化扁平上皮），皮膚付属器（皮脂腺，汗腺，毛嚢），神経組織，骨・軟部組織，呼吸器上皮や消化管上皮，甲状腺などがよくみられる。全卵巣腫瘍の10～20％を占め，胚細胞腫瘍の中では80～90％と最も頻度が高い^1）。80％は20～50歳の間に認められ，30歳前後にピークを有し，妊娠に合併することも多い。多くは片側性で，両側性は9～16％でみられる。肉眼的には単房性嚢胞性腫瘤であることが多く，嚢胞壁は比較的厚く角化扁平上皮に裏打ちされ，壁内に皮脂腺や毛包などを含む。嚢胞内に皮脂腺由来の黄灰白色で粥状の皮脂物やケラチン，毛髪などがみられる^2）。表皮と付属器が主体のときは皮様嚢胞腫（dermoid cyst）と呼ばれる。3～12％は多房性で，まれに充実性のこともある。無症状で偶発的に見つかることが多いが，腫瘤が大きいと腹痛や腹部膨満などの症状を示す。予後は良好だが，未熟奇形腫の検索，茎捻転や被膜破綻などの合併症予防，悪性転化の予防目的に外科治療を必要とする。近年，奇形腫に合併する自己免疫性脳炎が報告されており，統合失調症様症状がみられる^3）。卵巣腫瘍の摘出により劇的に症状が改善される。嚢胞内に脂肪を含む頻度が高く，画像上脂肪を同定することで特異的診断が可能である。通常は超音波検査で診断可能であるが，内容物が多彩なため診断に苦慮することもある^4）。CTは脂肪と石灰化を検出するのに有用であり，超音波と比較して感度（93～98％）が高い^5）。時に脂肪が毛髪や他の成分と混ざると，CT値が水と同じようにみえることがあり，注意が必要である^6）。MRIは優れた組織分解能をもち，純粋な脂肪から少量の脂肪成分を特定することができる。腫瘍内の純粋な脂肪は，T1強調像およびT2強調像で高信号，脂肪抑制T1強調像で信号が低下する。微視的な脂肪の存在は，gradient echo（GRE）法を用いたchemical shift imagingで確認することができる^7）。CTおよびMRIでは様々な画像所見およびサインが報告されている^8）（表1）。成熟奇形腫は良性腫瘍であり診断は比較的容易であるが，時に悪性腫瘍と紛らわしいことがあり，過剰な治療を回避するためにも術前診断は重要である。

Mature cystic teratoma（MCT）is a common neoplasm of the ovary that typically contains mature tissues of ectodermal, mesodermal, and endodermal origin. This tumor tends to affect younger women, its presentation ranges from pure cystic mass to complex solid cystic mass, and the detection of intratumoral fat component is the key diagnostic imaging feature. MCT can be associated with various complications and it demonstrates a wide spectrum of imaging findings. Associated complications include rupture, torsion, malignant transformation. MCT may also have unusual imaging features that can lead to misdiagnosis. These features may expand the differential diagnosis to include immature teratoma, monodermal teratoma, mature cystic teratoma with minimal or no fat, and collision tumor. The aim of this article was to highlight and describe the imaging features of unusual ovarian MCT.
