はじめに　Frozen elephant trunk（FET）法は弓部大動脈病変に対して広く用いられており，近年では分枝付き人工血管一体型FETであるThoraflex Hybrid（TH）［Terumo社］の使用も増加している1，2）．われわれはTHを使用したFET併用弓部大動脈置換術（TAR-FET）後に生じたFET末梢のマイグレーションに起因する大動脈瘤破裂に対し，胸部ステントグラフト内挿術（TEVAR）により救命した1例を経験したので報告する．

We report a case of migration of the distal end of the frozen elephant trunk （FET） after aortic arch repair with Thoraflex Hybrid, resulting in aneurysm rupture and successfully treated with thoracic endovascular aortic repair. In this case, the FET procedure was performed for an aneurysm complicated with subacute aortic dissection. Postoperative computed tomography （CT） showed that the fixation of the FET was shorter than expected, and the oversizing effect of the FET was reduced by remodeling of the dissected aorta. These factors may have affected the migration of the FET.
