While total aortic arch replacement with frozen elephant trunk (FET) is a common procedure for ruptured thoracic aortic aneurysm, there is a lack of documented cases regarding postoperative bleeding attributed to FET endoleak necessitating chest re-exploration. We present a case of postoperative bleeding from a ruptured site following emergent total aortic arch replacement with FET for an impending rupture of a distal arch aortic aneurysm (71 mm), which was successfully controlled with thoracic endovascular aortic repair.
