破裂性弓部大動脈瘤に対する全弓部大動脈置換，オープンステントグラフト内挿術の直後に生じた再破裂を追加ステントグラフトで制御した1例

Postoperative Bleeding Attributed to Frozen Elephant Trunk Endoleak Which Is Controled by Thoracic Endovascular Aortic Repair：Report of a Case

はじめに　胸部大動脈瘤破裂に対する全弓部大動脈置換術＋frozen elephant trunk（FET）はしばしば行われる術式であるが，術後FETからのエンドリーク（EL）による出血再開胸の報告はない．われわれは，弓部囊状瘤（71 mm）の破裂に対して準緊急で行った全弓部大動脈置換術＋FETの術後，破裂孔からの出血に対し，胸部大動脈ステントグラフト内挿術（TEVAR）の追加により止血しえたので報告する．

While total aortic arch replacement with frozen elephant trunk （FET） is a common procedure for ruptured thoracic aortic aneurysm, there is a lack of documented cases regarding postoperative bleeding attributed to FET endoleak necessitating chest re-exploration. We present a case of postoperative bleeding from a ruptured site following emergent total aortic arch replacement with FET for an impending rupture of a distal arch aortic aneurysm （71 mm）, which was successfully controlled with thoracic endovascular aortic repair.
