はじめに 三尖弁閉鎖不全症(TR)は多くが左心系疾患や肺高血圧に伴う機能性のものであり,進行すると右心不全症状を呈し,生命予後を悪化させる1).薬物治療に抵抗性の重症例に対しては外科治療が推奨される.一方,再生不良性貧血(aplastic anemia:AA)は骨髄の造血幹細胞の障害により汎血球減少をきたす疾患であり,人工心肺を使用する開心術においては出血傾向や易感染性が大きな問題となる.われわれはAAによる汎血球減少を合併した重度TRに対し,周術期に厳格な血球管理を行い,三尖弁輪縫縮術および右房縫縮術を施行した症例を経験したので報告する.
Performing open-heart surgery in patients with aplastic anemia (AA) presents significant challenges due to hemorrhagic tendency and increased infection risk. We report a successful case of tricuspid annuloplasty (TAP) and right atrial plication in a 76-year-old female with severe tricuspid regurgitation (TR) and right heart failure, complicated by AA. Preoperatively, the patient exhibited marked cardiomegaly [cardio-thoracic ratio (CTR) 81%], a giant right atrium, and severe TR, necessitating strict perioperative hematological management. Granulocyte-colony stimulating factor (G-CSF) was administered, alongside planned platelet and red blood cell transfusions. The surgery proceeded with no postoperative infectious complications. Postoperatively, heart failure symptoms significantly improved [New York Heart Association (NYHA) Ⅰ], and cardiomegaly ameliorated (CTR 62%) at five months. This case highlights that safe cardiac surgery in AA patients is achievable with meticulous perioperative management.
