はじめに　鎖骨下動脈瘤は末梢動脈瘤の中でもまれな疾患であり，瘤の部位により手術アプローチ，方法の適切な選択が必要となる．われわれは巨大右鎖骨下動脈瘤に対しハイブリッド治療を行い，良好な結果を得たので報告する．

An 85-year-old woman was diagnosed with intrathoracic right subclavian artery （SCA） aneurysm with a maximum diameter of 50 mm located at the proximal portion of the right SCA and adjacent to the vertebral artery and right internal thoracic artery （RITA）. She was successfully treated with the right carotid-left axillary bypass followed by covered stent-graft deployment from the brachiocephalic artery to the right SCA to exclude the origin of the aneurysm. The postoperative course was uneventful, and computed tomography （CT） conﬁrmed the patency of the bypass graft and complete exclusion of the aneurysm, which decreased in size. We considered less invasive endovascular procedures and alternative treatment options for right SCA aneurysms.
