感染症診療における遺伝子検査活用のコツとピットホール

Applications of molecular diagnostic techniques for infectious diseases

近年，感染症の診療において，遺伝子解析技術は分離菌株の迅速な菌種の同定のみならず，検体から直接，微量な病原体を検出・同定する際に追加の検査として利用されている．筆者らは，この約6年間継続して，全国の病院において日常検査法で診断できなかった830症例以上の臨床検体を精査してきた．本稿では，“どのような状況で遺伝子検査を活用するか”を把握してもらうべく，解析した代表的な症例の病態や病因診断までのプロセスを紹介しながら，遺伝子検査の活用のコツとピットホールについてまとめてみたい．遺伝子解析技術を用いた感染症の診断においても，臨床医との緊密なコミュニケーションにより得られた情報を熟慮しながら解析に当たることが重要であることを強調したい．

In recent years, molecular microbiology techniques have proven to be a useful supplement to conventional assays not only for characterizing organisms from culture, but also for directly detecting pathogens from clinical samples.　This review is intended to explore the application of molecular diagnostic techniques for infectious diseases in certain clinical contexts.　The overall goal of this article is to assess how these techniques can be integrated to enhance diagnostic capabilities for infectious diseases.　Finally, we should put emphasis on the point that close collaboration between physician and clinical microbiologist is required in all cases where molecular diagnostics is contemplated because assays need to be individualized according to the clinical settings.
