I．はじめに
上顎癌の広範囲進展例でも癌の根治と同時に形態と機能を保存して社会復帰させる目的で，放射線療法と局所化学療法および開洞手術も含めた局所清掃の三者併用療法を実施している。現在私どもの実施している三者併用療法は，手術を前提とした術前処置でなく，BAR療法とも違う，TNM分類で治療法をきめるのでなく，個々の症例の臨床所見を重要視して腫瘍ならびに周囲健常組織の反応に応じて開洞減量操作と局所清掃を行ない，1例1例の治療配量をきめてゆくことである。癌に対する照射，動注の直接作用も欠かせないが，三者併用療法の成功のかぎは腫瘍がいきいきしているかおとなしくなりだしたかによつて，これを除去する治療操作と残存腫瘍を健常組織のself-defence mechanismにゆだねて癌の根治をはかることで，毎日毎日注意深く観察し十分に臨床像を把握することが必要である。
今回は治療中の腫瘍の走査型電子顕微鏡（以下走査電顕と略す）による細胞レベルで観察した所見について報告する。
The surface view of malignant maxillary tumors (squamous cell carcinoma, undifferentiated adenocarcinoma and fibrosarcoma) was observed in detail by means of scanning electron microscope.
The squamous cell carcinoma and undifferentiated adenocarcinoma showed a disorderly growth pattern. Their cell surfaces were irregular and roughly granulated. Metastasized fibrosarcoma showed abundant collagen fibers with decreased number of tumor cells.
Intrarterial infusion of 5-FU and BUdR with 60Co irradiation in the maxillary region induced necrosis of tumor tissues. A large number of lymphocytes were invariably noted among the necrotized tumor cells. These lymphocytes were of T-type in appearance and it is probable that these cells might play an important role in acquiring cellular immunity that may lead to the destruction and disappearance of the malignant tumor cells.
