気管支喘息に続発した縦隔気腫の3症例

SPONTANEOUS PNEUMOMEDIASTINUM, A REPORT OF THREE CASES

　I．はじめに

　気管支喘息の合併症として肺気腫，無気肺，気管支拡張症などは多いが，縦隔気腫や皮下気腫の続発した報告は本邦において少ない。外国では1800年代に小児喘息に続発した報告があり，その後も散発的に報告されている。この合併症，すなわち続発性の縦隔気腫の発生機序についてはMacklin^3）らによつて実験的な根拠をもつ説明がなされている。

　耳鼻咽喉科領域においても，気管切開後に頸部に皮下気腫の併発することはしばしば経験することであり，まれに縦隔気腫にまで発展することもある^10）。そのほか頸部の外傷や手術，気管・気管支・食道などの損傷のさいにもみられ，縦隔気腫に遭遇する機会は多い。しかし，気管支喘息に続発した縦隔気腫の報告はきわめて稀である。

　今度，われわれは気管支喘息に続発した縦隔気腫と皮下気腫の3例を経験したので，若干の考察をくわえて報告する次第である。

　Three cases of spontaneous pneumomediastinum with subcutaneous emphysema which arose as a complication following a severe asthmatic attack, are reported. This disease appears to be rare in this country and only a few reports are seen.

　Animal experimentations suggest that spontaneous pneumomediastinum is a development that follow after an attack of severe respiratory distress, so, patients showing subcutaneous emphysema after an asthmatic attack call for a careful physical examination. X-ray of the chest is indispensable.

　Conservative treatment is effective in most cases. However, cervical mediastinotomy with or without tracheotomy may be necessiated at times which could be a dramatic lifesaving measure in some cases.
