I．はじめに

　交通外傷による急性硬膜外血腫とそれに対する血腫除去術後に，外傷性中硬膜動脈偽性動脈瘤の発生を認めた1例に対して，開頭術を用いず血管内手術によって治療を行い動脈瘤を消失させ得た．本疾患はその頻度の少なさから見逃されやすく注意を要する．本症例では出血を繰返しながら硬膜外血腫が緩徐に増大してきたと思われるが，神経学的脱落症状の発現前にそれを未然に防ぎ得た．それに対する治療として，血管内手術は，侵襲が少なく患者に対する負担も軽いが，その反面，再開通の可能性もあり，反復した治療も必要となり得る．

　われわれは，本症例を血管内手術のみによって完治させ得たことから，同疾患に対する治療として，考慮されるべき一つの選択肢となりうるものと考える．

A case of traumatic pseudoaneurysm treated by endovascular surgery is reported.

The patient was a 20-year-old female. Acute epidural hematoma due to a traffic accident wasremoved.She was transferred to our hospital in the chronic stage after the operation, because traumaticpseudoaneurysm of the middle meningeal artery had appeared in the same location and had increased insize.

We were able to eliminate the aneurysm by endovascular surgical treatment without craniotomy.

Endovascular surgical treatment with embolization was less invasive to this patient with traumaticpseudoaneurysm of the middle meningeal artery than the usual surgical procedure.
