I．はじめに

　圧可変式シャントバルブ（以下SOPHYバルブと略す）は種々の長所を有しており，近年これを使用する機会が増加している．しかし，症例数が増えるとともに問題点も報告されるようになった^1-6）．今回われわれは本システム使用後に，SOPHYバルブが胸部皮下ポケットで反転したことに気付かず，圧設定を誤り，症状の増悪を来した3症例を経験した．かかる医原性合併症はこれまでのところ報告されておらず，本シャントシステム設置時の留意点や，必要と思われる改良点につき検討したので報告する．

Using a Sophy-valve system, complications after V-P shunt have been encountered in 3 cases.

Turning over of the valve in a subcutaneous pocket would lead to a false judgement of setting pressure, and to serious iatrogenic complications. The valve seems to turn over only in the condition of subcutaneous place-ment in the chest. Probable causes are loose chest tissue, difficulty of tight fixation, and round and smooth shape of the valve. It should be kept in mind that the Sophy-valve can spontaneously turn over when it is placed in the chest.Careful examination of the chest X-ray can help to judge whether the valve is in appropriate position or not.
