Ⅰ．はじめに

　頭部外傷急性期の患者管理に意識レベルの推移把握が重要であることは繰り返し強調されているが^{7，9，11）}，これを受傷後経時的に追跡し各時間ごとの意識状態と予後を比較した報告はみられない．この原因の一つに意識に関連した言葉の定義が明確でないことがあげられる．筆者らは今回外部刺激に対する患者のmaximum responseにより判定した意識状態と患者の予後を比較し，この判定法が重症頭部外傷患者の予後推定に有用であると老えたので報告する．

　1) All cases of severe head injury (145 cases) admitted during 1969-1971 were reviewed and the state of consciousness seen at set times after the injury was correlated with ultimate fate of patients.

　2) The state of consciousness defined in operational terms as the maximum response to applied external stimuli seems to correlate well with the prognosis of recovery of patients, and would he useful as one of the parameters in estimating progress or deterioration and therefore in managing the severely head injured.
