AANS 79th Annual Scientific Meeting（2011年4月9～13日）--AANS杉田虔一郎記念国際シンポジウムに寄せて・1--真の脳神経外科指導者育成法

Neurosurgical Leadership: Lessons Learned

　2011年4月9日から13日にかけて，アメリカ，デンバーにて開催されたAmerican Association of Neurological Surgeons（AANS）の第79回総会の会長を務めさせていただいたことは大変名誉なことでした．AANSは米国内では脳神経外科領域の単独診療科で構成される最大級の組織であり，会員の対象は全世界の脳神経外科医となっています．会員は現在，それぞれがさまざまな分野にわたっていますが，総勢で8,000人を超えています．この組織は，各種の専門委員会により構成されており，会員にとってさまざまな有益となる情報を発信しています．例えば，学会誌であるJournal of Neurosurgeryには脳神経外科学の学術論文を収載し，その他にも医療訴訟におけるサポート，多様なオンライン教育の場の提供，脳神経外科学における最新の重要な知見を提供する総会の開催など，さまざまな役割を果たしています．

　さて，デンバーにおけるAANSの総会は私にとって特別な総会であり，それにはいくつか理由があります．まず，名古屋大学脳神経外科の若林俊彦教授，信州大学脳神経外科の本郷一博教授のご協力により，AANS杉田虔一郎記念国際シンポジウム（The AANS-Kenichiro Sugita International Symposium）を開催できましたことは，私にとって大変喜ばしいことでした（Fig. 1）．杉田教授は私にとって最も重要な指導者の1人であります（Fig. 2）．私は彼から，微小脳神経外科手術（microneurosurgery）の芸術性と鍛錬について非常に多くのことを学びました．杉田教授のご指導による多くの技術は，今日でも，私が実際に脳神経外科手術において駆使しているものです．事実，私は手術において複雑な手順を踏む場合，今も杉田教授によって描かれた素晴らしい微小脳神経外科解剖図と手術症例を参考にしています²⁾．彼が中大脳動脈瘤のクリッピング術の直後に，その詳細を描いた時，私はその描写を観察することで，脳裏に克明に刻み込むという恩恵を受けることができました．

　It was my great pleasure and honour to serve as the President of the American Association of Neurological Surgeons (AANS) for its 79^th Annual Meeting in Denver April 9 ― 13^th,2011. The AANS is the largest single country organization for neurosurgeons in the world. Its membership is now over 8,000 neurosurgeons in the various categories. In addition,the AANS prides itself with offering numerous important benefits to members including the Journal of Neurosurgery,the journal of record for academic neurosurgery in the world,defense by the Professional Conduct Committee for neurosurgeons who are charged with egregious lawsuits from testimony by other AANS neurosurgeon members,numerous on-line education offerings,and a superb annual meeting which presents the latest and most significant findings in neurosurgery to the fore each year.

　The Annual Meeting of the AANS in Denver was a very special meeting for me for a number of reasons. For one,I was so pleased to organize a special AANS-Kenichiro Sugita International Symposium with the help of Professors Toshihiko Wakabayashi,Chairman of the Department of Neurological Surgery at Nagoya University,and Kazuhiro Hongo,Chairman of the Department of Neurological Surgery at Shinshu University (Fig. 1). Professor Sugita was one of my most important mentors in neurosurgery (Fig. 2). I learned so much from him about the art and discipline of microneurosurgery. Many of the technique he espoused regarding operative neurosurgery I use to this day in my own practice of neurosurgery. In fact,I still refer to Professor Sugita's wonderfully illustrated atlas on microneurosurgical anatomy and operative cases whenever I am about to perform a complex procedure ²⁾. I had the great privilege of watching a canvas come to life as he painted the details of the clipping of a complex middle cerebral artery aneurysm immediately after the case. Another very important reason why the Denver meeting was so important to me was the awarding of the AANS International Lifetime Achievement Award to Professor Shigeaki Kobayashi (Fig. 3),Professor Sugita's associate at Shinshu University.
