Biomechanics of human cerebral arteries.

I．序論

　血管壁は主として弾性繊維，膠原繊維，平滑筋の3要素から構成されている。このうち弾性繊維と膠原繊維は血管内圧などの外力によって変形を生じる受動的構成要素であり，平滑筋はその活性化により応力の発生あるいは血管外径の減少をきたす能動的構成要素である^{5,9,14,18,25,26,35,45,48,49）}。血管壁におけるこれら各要素の力学的関連性は必ずしも明らかにされてはいないが，平滑筋は弾性繊維，膠原繊維といった血管結合織成分を媒介として，その機能を発現するものと考えられている。これら各要素が血管壁に占める割合や配列は血管部位によって異なり（後述），したがってこれらによって規定される血管壁の変形特性も異なるものと考えられている^{9〜12,36,47）}。

　これら血管壁の変形特性に関する研究は，概して血管壁のstripやring状試料を研究材料に用いている場合が多く，詳細な材料力学的解析にはいくつかの方法論的問題点を内在している^{23,24,26,28,51）}。すなわち実験時に試料に負荷される張力あるいは応力が生理的な範囲か否かの確認が困難であること，平滑筋活性時に生じる等尺性張力は必ずしも等圧性収縮とは一致しないこと，生じた張力や血管収縮は血管壁に負荷される歪や応力に依存して変化すること，さらに本来円筒状である血管試料をstripやring状試料とする操作自体の影響が明らかになっていないこと，などである。

Human intracranial (vertebral and basilar) and extracranial (internal and common carotid, and vertebral) arteries were studied under passive and active conditions of smooth muscle component. Intracranial vertebral arteries subjected to subara-chnoid hemorrhage were also employed to study the mechanics of cerebral vasospasm. Stiffness parameter(β), incremental elastic modulus(Einc), wall thickness ratio (T/Ri), tangential wall stress(δ) and tangential mid-wall strain(εm) were calculated from the pressure-diameter relations observed in the pressure elevating process, which are given by the following equations :
