脳血管外科におけるTranscranial Dopplerの有用性について

The Prospect of Usefulness of Transcranial Doppler Sonography in Cerebrovascular Surgery

I．はじめに

　従来より頭蓋は超音波の頭蓋内への侵入を阻み超音波診断の"壁"となっていた．しかし最近になり，経皮・経頭蓋的に頭蓋内主要血管の血流が直接検出できる超音波ドプラ装置が開発され，すでに臨床応用の報告も散見される^1,2,6）．脳血管外科的治療に際し血流動態検査法として，この経頭蓋的超音波ドプラ血流計測（transcranialDoppler sonography）の有用性を検討したので報告する．

In 30 patients undergoing neurosurgical intervention, the authors evaluated the hemodynamics in the circle of Willis by transcranial Doppler sonography. By avoid-ing confusion with collateral effects, the transcranial Doppler sonography yielded direct and more significant information concerning the intracranial hemodynamics than extracranial Doppler sonography. Therefore, it made possible to detect intracranial occlusive lesions with less false findings. It gave us more accurate in-formation on the effectiveness of reconstructive vascu-lar surgery.
