前頭葉の損傷では，独特の感情障害（広くは情動変化とそれに基づく行動異常）のみが選択的に出現する場合がある。そして前頭葉穹窿面が障害された場合は無関心・無感情となり発動性の低下，感情の平板化などが目立ち，一方，前頭葉眼窩面が障害された場合は，児戯性・多幸・モリァといった性格変化がおこり，脱抑制，非社会的逸脱行動が目立つようになると言われている。視床や淡蒼球の限局性病変でもこれらの症状が選択的に出現する場合がある。このような症例の機能画像では前頭葉の機能低下が明らかになっている。前頭葉を介する複数の神経回路における各回路の遮断で前頭葉自体の損傷と同様の回路特異的な障害が生じると考えられている。変性性痴呆症の一つであるピック病でも同様の情動変化が出現する。この疾患では前頭葉のみならず，側頭葉前方部，尾状核，扁桃体といった部位が障害されるが，単に部位ではなく，系（システム）の損傷に基づいて症状が生じていると考えればわかりやすい。

The frontal lobe plays a critical role in human behavior with emotion. Cases of the frontal convexity syndrome are apathetic, indifferent, or unmotivated, while cases of the orbitofrontal syndrome are disinhibited, socially inappropriate, euphoric, or jocular. Cases of the medial frontal syndrome are akinetic. These distinct frontal lobe neurobehavioral syndromes corresponds to one of the currently recognized three prefrontal-subcortical circuits. The behavioral syndromes following frontal lobe lesions are recapitulated with lesions of subcortical member structures of the circuits, such as striatum, globus pallidus, and thalamus.
