Lichen amyloidosusについて

LICHEN AMYLOIDOSUS

　Rokitansky^1）は内臓に特異な均質性物質の沈着する疾患をはじめて記載したが，Virchow^2）はその物質がヨードに対して澱粉と同様の反応態度を示すことからAmyloid-substanz （類澱粉物質）と命名した，このような全身性に類澱粉物質の沈着する疾患が数多く報告されるにしたがつて，時には皮膚および粘膜にも同様の物質が沈着することが知られてきた．一方Kenedy^3）は，acrodermatitis atrophicans diffusaと類似の症状を呈する皮膚限局性の類澱粉沈着症を報告したが，Königstein^4）は，紅色苔癬およびVidal苔癬を思わせる同症を記載し，かかる症例に対しFreudenthal^5）はlichen amyloidosusと命名した．その後Gottron^6），Marchioniniら^7）によつて種々の異なつた型の限局性類澱粉症が報告されたが，Röckl^8）はこれらの各種の限局性類澱粉沈着症を原発性と続発性に大別し，前者にtumorförmige Amyloidose（Holzmann u．Skeer），amyloidosis cutisnodularis atrophicans（Gottron），lichen amyloid-osus（Freudcnthal），poikilodermieartige Amyloidose（Marchionini u．John），maculöse Variante（Palitz u．Peck）の5型を含めた．またGracia-nskyら^9）は，amyloïdose papleuse à type de lichen amyloide de Freudenthal, amyloidose moniliforme de Perassu, amyloïdose macleuse de Palitzet Peckの3型をあげており，Ruszczakら^10）は，次のような分類を試みている．

A．primäre Veränderungen

Three eases of this disease were reported.

Case 1: A 21-year-old woman with brownish-red or dark brown lichenified plaques on the upper part of the back.

Case 2: A 29-year-old man with crops of brownish red papules on both the lower legs.

Case 3: A 27-year-old woman with lichen Vidal-like brownish-red plaques on the central part of the back.

Histologic specimens of the 3 cases showed the circumscribed amyloid deposition in the papillae just beneath the epidermis, which showed metachromasia by the crystal violet stain and yellow fluorescence by the thioflavin T stain.

The ODT of 0.025% Fluocinolone acetonide cream introduced marked improvement of the lesions.
