　はじめに

　通常強度の自発眼振を認めるようなめまい発作中には患者の苦痛が強く頭位眼振検査もままならない場合もある。我々が今回報告する症例は，めまい発作中にも関わらず患者が温度刺激検査施行時に苦痛を訴えず，むしろ検査を望んだのでこのような自発眼振の著明な時期に検査を反復することが可能であった。このような症例に遭遇することは稀であると思われたので，臨床経過や検査結果について詳細に報告し，若干の考察を加える。

A-48-year-old female received bithermal caloric test during vertiginous attack of Meniere's disease. Before the caloric stimulation, spontaneous nyst-agmus directed to the left side was observed and the function of her affected ear (left side) was evaluated as the canal paresis (the data wascaluculated according to Jongkees' formula). But the response elicited by irrigation of ice water at the same time was strongar than the response elicited at other time. So the authors speculated that the spontaneous nystagmus directed to the side of the affected inner ear was caused by the stimulated situation of this ear.
