はじめに

　転移性甲状腺癌の頻度は甲状腺癌の1〜3％と低い^1,2）。既存の腫瘍内への転移となるとさらに稀である。われわれの渉猟しえた範囲では，海外と併せても30例ほどしか報告されていない^2〜8）。今回，肺腺癌の精査でPET/CT検査を行ったところ甲状腺右葉に腫瘍を認め，穿刺吸引細胞診の結果，悪性腫瘍と診断したため摘出術を行い，甲状腺濾胞癌内に転移した肺腺癌と診断した症例を経験したので，若干の文献的考察を加えて報告する。

　Tumor-to-tumor metastasis is extremely rare in the thyroid gland. We report a case of lung adenocarcinoma metastasizing to thyroid neoplasm and review the related literature. A 74-year-old male patient was diagnosed with lung adenocarcinoma. PET/CT showed a hypermetabolic lesion in the right lobe of the thyroid gland and right upper lobe of the lungs. Fine needle aspiration of the thyroid tumor was performed, and cytological examination showed malignant findings. Histological examination after radical thyroidectomy revealed lung adenocarcinoma metastasizing to a follicular neoplasm.
