はじめに

　CD8^＋ cytotoxic T cell lymphomaは，WHO2016分類で末梢性T細胞性リンパ腫（peripheral T cell lymphoma：PTCL）に分類される疾患であり^1），報告例はきわめて少ないが予後は不良とされている。鼻腔に初発した場合には，節外性T/NK細胞リンパ腫に類似した顔面正中部の破壊性病変や鼻炎様症状で耳鼻咽喉科を受診することがある。一方，通常の病理検査では診断が難しく，詳細な免疫学的特性検査を行って診断されることが少なくない。今回，系統立ったリンパ腫解析システムによって診断しえた鼻腔初発のCD8^＋ cytotoxic T cell lymphomaの1例を経験したので報告する。

　We experienced a case of nasal first CD8^＋ cytotoxic T cell lymphoma with difficulty in diagnosis. Patients' symptoms were left nasal bleeding, nasal congestion, and post nasal pus. Tumor was found in the back of the nose and nasal cavities, but a biopsy was performed in other otolaryngologist, but it did not lead to diagnosis. About one year and six months after the first visit, we confirmed the diagnosis as a result of analysis by systematic lymphoma analysis system. For diagnosis of lymphoma, it is desirable to consider systematic pathological examination analysis.
