I．はじめに

　Chlamydia trachomatisは性行為感染症（STDs；sex—ually transmitted diseases）の病原体として再び脚光を浴びるようになった。そしてC. trachomatisは感染した産道を通る時，こどもに感染し乳幼児のトラコーマや封入体結膜炎，そして無熱性肺炎の原因になることが報告されている。中耳炎に関しては，Tippleら^1）がC. trachomatisによる無熱性肺炎で入院した生後5か月未満の患児37名について調べ，半数に中耳疾患があり，そのうち鼓膜切開を行った11名中3症例で中耳貯留液よりこの病原体が見つかったことを報告している。しかしHammerschlagら^2）は9か月から8歳までの滲出性中耳炎児68名の中耳貯留液とアデノイド組織のC. trachomatisを検索したが，分離できたのはたった1例のアデノイドからで，1歳以上の子供ではC. trachomatisは滲出性中耳炎の病原体として大きな役割は演じていないのではないかと述べている。

　われわれは最近C. trachomatisが原因と思われる滲出性中耳炎を3例経験し，滲出性中耳炎の成因を考えるうえで興味ある知見を得たので報告し，クラミジア感染に関する問題点について考えてみた。

The spectrum of clinical disease caused by C. trachomatis is rapidly expanding. Otitis media with effusion (OME) has been linked etiologically to this organism. We report three cases of OME, and in a patient with acute OME, the organism was cultured from the middle ear effusion and adenoid tissue. C. trachomatis was detected only by immunofluorescence method, not by culture in other two patients with chronic OME. Anti-bodies to serotype C and J C. trachomatis were found in the effusion in one of them. Immune complex appears to play an important role of generating OME.
