Ⅰ．はじめに

　喉頭浮腫は耳鼻咽喉科の日常診療でしばしば遭遇する病態であり，その原因は多岐にわたる。喉頭浮腫に対しては，緊急気管切開を要する場合と，経過観察し保存的に治療する場合とがあり，その判断が重要となる。今回，上大静脈症候群（superior vena cava syndrome：以下，SVCS略す）により両頸部および顔面の腫脹，そして喉頭浮腫をきたして緊急気管切開術を施行した症例を経験したので若干の文献的考察を加えて報告する。

　A 79-year-old man developed a swelling of his face and neck after upper respiratory inflammation. He needed an emergency tracheostomy because of a severe edema of the larynx and pharynx. He suffered from an Alzheimer-type dementia, and he could not keep the resting position. CT-scan and MRI were not useful in the present case. On the other hand, color doppler ultrasound revealed thrombosis of the bilateral internal jugular vein. The causative diseases of an edema of the larynx are numerous. Superior vena cava syndrome could be one of them.
