　51歳の女性が，7か月前からの右眼球結膜の腫瘤で受診した。9か月前から左乳房の湿疹様変化があり，Paget病と診断され，1週間前に単純乳房切除術を受けた。右眼の下方球結膜から下円蓋にかけて鮭紅色の境界明瞭な楕円形の腫瘤があった。腫瘤の病理組織学的所見として，小型リンパ球組織が増殖し，濾胞様構造が一部分残っており，炎性偽腫瘍のリンパ腫型と診断した。乳房Paget病の転移ではなかった。眼組織がたまたま慢性炎症のある結膜のリンパ球を刺激して炎症性変化が続発し，今回のような病変が生じたと推定した。この2疾患に関係する病態生理は不明であった。

A 51-year-old woman presented with a tumor in her right eye since 7 months before. An eczematous lesionhad been present in her left breast since 9 months before. She had been diagnosed with Paget disease and underwentsimple mastectomy. Her right eye showed a well-demarcated salmon-pink tumor in the lower bulbar conjunctiva extendingto the lower fornix. The resected tumor showed, histopathologically, proliferation of small lymphocytes mixed with follicularstructure.These features led to the diagnosis of lymphoid type of inflammatory pseudotumor. Metastasis of Paget diseasein the breast to the conjunctiva could be ruled out. It appeared that the conjunctival lymphocytes were stimulated bymalignant cells of Paget disease to form a chronic inflammatory lesion in the conjunctiva.
