　麻酔による患者への侵襲を最小限にする目的で，1990年4月より点眼およびテノン嚢内麻酔のみによる白内障眼内レンズ手術を導入し，現在までに1,800件以上の症例を経験した。2％リドカイン約1mlを1時の位置のテノン嚢内に注入するのみであり，眼球運動は抑制されないが，麻酔による疼痛が少なく血圧の上昇や不整脈の出現などがないこと，麻酔による合併症がなく重篤な術中合併症もないこと，少量の投与で確実な鎮痛効果が得られること，60分以上の効果があり初心者の手術にも利用できることなどから，テノン嚢内麻酔は従来の球後麻酔に代わる有用な麻酔法と思われる。

From a review of 1,800 cataract surgeries, we could confirm the usefulness of topical injection of 1ml of 2% lidocaine into the Tenon's capsule. This procedure was safer than conventional retrobulbar anesthesia because of absence of pain and lesser incidence of complications including retrobulbar hemorrhage. It could also be learned with ease and be administered even by a beginner surgeon. It was better tolerated by the patient and did not induce rise of systemic blood pressure or arrythmia. While this procedure does not repress ocular motility, this feature did not noticeably affect the surgical mani-pulation for cataract and intraocular lens implanta-tion.
