COVID-19ワクチンmRNA-1273接種後に急性前部ぶどう膜炎と視神経乳頭腫脹を発症した1例

要約　目的：COVID-19ワクチンmRNA-1273接種後に急性前部ぶどう膜炎と視神経乳頭腫脹を発症した症例の報告。

症例と所見：17歳女性がmRNA-1273の1回目のワクチン接種翌日からの発熱および左眼の霧視，充血，眼痛を自覚し，近医から紹介され受診した。視力は右（1.5），左（1.0p）で，左眼に毛様充血，前房細胞，前房フレア，微細な角膜後面沈着物，視神経乳頭の発赤と腫脹を認めた。左眼の前部ぶどう膜炎および視神経乳頭腫脹と診断し，ステロイドと副交感神経遮断薬の点眼にて治療後，視神経乳頭の発赤と腫脹を除いて軽快した。1回目から4週後の2回目のmRNA-1273のワクチン接種の翌日にも同様に，発熱および両眼の充血，左眼の霧視を自覚し来院した。検査では，視力は右（1.5），左（1.2），左眼の前房細胞，前房フレア，前部硝子体細胞，前部硝子体フレア，視神経乳頭の発赤と腫脹を認めた。ステロイドと副交感神経遮断薬の点眼にて治療後，視神経乳頭の発赤と腫脹以外の所見は改善し，現在は無治療で経過観察中である。

結論：COVID-19ワクチンであるmRNA-1273接種が原因と考えられる急性前部ぶどう膜炎と視神経乳頭腫脹を発症した症例を報告した。

Abstract　Purpose：To report a case of acute anterior uveitis and optic disc swelling post vaccination of mRNA-1273 for COVID-19.

Case and findings：A 17-year-old woman presented with blurred vision, injection, and pain, in the left eye continuing from the day after the first vaccination of mRNA-1273. Visual acuity was（1.5）and（1.0p）in the right and left eye, respectively. The left eye showed ciliary injection, anterior chamber inflammation with cells, flare, and fine keratic precipitates, and optic disc swelling. Upon diagnosis of anterior uveitis and optic disc swelling, steroid and atropine eye drops were administered. Shortly thereafter, her symptoms and signs except for optic disc swelling were gone. The day after the second vaccination of mRNA-1273, she had a fever, injection of both eyes, and blurred vision in the left eye. The left eye showed mild anterior chamber inflammation, considerable vitreous inflammation with cells and flare, and optic disc swelling. She was treated with the same regimen, and her symptoms and signs except for optic disc swelling recovered. The patient is currently being followed up without treatment.

Conclusion：We reported a case of acute anterior uveitis and optic disc swelling related with COVID-19 vaccine mRNA-1273.
