　血管脂肪腫は成熟した脂肪細胞と毛細血管様の血管増生が混在した腫瘍であり，局在部位によって非浸潤型と浸潤型に分けられる．浸潤型血管脂肪腫は稀な腫瘍であり，今回われわれは筋肉内発生の1例を経験した．症例は69歳の男性，左腹斜筋内に再発性腫瘍を認めた．病理診断で浸潤型血管脂肪腫と診断されたため，再発腫瘍に対し周囲の正常組織を含めて広範切除を行った．浸潤型血管脂肪腫は筋肉内に発生し局所浸潤傾向が強いため，腫瘍を辺縁で切除した場合再発する可能性が高く，広範切除術が推奨される．

　Angiolipomas are benign soft-tissue tumors composed of mature fatty elements and blood vessels in varying proportions.　They are classified into two histological types, infiltrating and non-infiltrating, and infiltrating angiolipomas are rare.　Here we report the case of a 69-year-old man with a recurrent tumor of the left abdominal wall.　The tumor was localized in the oblique muscles of the abdomen.　The pathological diagnosis was infiltrating angiolipoma.　The tumor was excised widely with the surrounding tissues.　Infiltrating angiolipomas are unencapsulated tumors that exhibit an infiltrative growth pattern.　Because of the significant risk of recurrence, wide excision is recommended.
