中高年者に対する解剖学的二重束前十字靱帯再建術の臨床成績―若年群との比較検討

Clinical Results of Anatomical Double-bundle Anterior Cruciate Ligament Reconstruction in the Middle-aged and Elderly. A Comparison Study with a Young Group

　背景：近年，スポーツ人口の高齢化とともに中高年者の解剖学的二重束前十字靱帯再建術が施行されるようになった．

　対象と方法：解剖学的二重束前十字靱帯再建術を施行した40歳以上である中高年群(19例)と若年群(72例)との間で，術前の臨床評価，合併損傷，術後成績について比較検討を行った．

　結果：術後膝前方安定性，pivot shift test陽性率，Lysholm score，伸展・屈曲筋力患健比は各群間で有意差はなかった．

　まとめ：中高年群の解剖学的二重束前十字靱帯再建術の臨床成績は若年群と比較して遜色なく良好であると考えられた．

　Background：Anatomical double-bundle anterior cruciate ligament reconstruction (DB-ACLR) is being increasingly performed in middle-aged and elderly patients.　The purpose of this study was to compare the clinical results of DB-ACLR in a group of middle-aged and elderly patients and a group of young patients.

　Method：A total of 91 reconstructed knees were assessed one year after the operation.　In this study "middle-aged and elderly" was defined as 40 years of age and over.　We divided the patients into two groups：a young group (72 knees) and a middle-aged and elderly group (19 knees) and compared the two groups in relation to the cause of the ACL injury, waiting period before the operation, Lysholm score, anterior laxity, results of the pivot shift test, meniscus injury, osteoarthritic change, and knee flexor and extensor muscle strength.

　Outcome：There were no significant differences between the two groups in waiting period before the operation, Lysholm score, anterior laxity, results of the pivot shift test, meniscus injury rate, and knee flexor and extensor muscle strength.

　Conclusion：We concluded that DB-ACLR is an effective method of treating middle-aged and elderly patients.
